Safety is dependent on doing things right

Not doing maintenance right can lead to disaster.

 Take a lesson from the Cortez mine when truck 280 burned. It was only a couple years old. The wrong pressure rated hydraulic hose was used as a replacement. The hose ruptured spewing hydraulic fluid over the rear wheels/drive train. The hydraulic fluid caught fire, spread to the tyres and the truck went up in flames.
